Community Support Services for People with Mental Health Problems

Ambient Support

This service aims to maximise people's independence and chance to lead a valued life within their local community. Support is provided on a 1:1 basis in the client's home.
Flexible support is offered for:-
* individuals living in the community with severe and enduring mental health problems, including complex needs and dual diagnosis;
* people who need support with daily living activities and who may be able to develop skills with 1:1 support;
* people who become socially isolated without regular contact;
* people who need support to manage their medication.
This support model is also used to help people with learning disabilities, alcohol and substance misuse and social support needs.

* Support can be provided from 7am to 10pm, 365 days a year and is tailored to the needs of the individual. * There are two types of flexible support: short term intervention, time limited support with people expected to move on within 6-8 weeks;
intensive and higher level support where people move on from the service within 6 months.
* There is a team of specialist staff including Support Workers, Occupational Therapists (OT) and OT Assistants to provide the service.
*REFERRALS - by Community Mental Health Teams but individuals wishing to buy the service via direct payments can self-refer. Referral forms are available at Community Mental Health Centres throughout the borough or by calling Ambient Support.
