Simply Connect Bromley is a Social Prescribing service for the borough of Bromley, managed and delivered by Community Links Bromley.

We’re on a mission to support older people and their carers to find local community services, activities and support that can really help you with whatever issue you’re struggling with at the moment.

Maybe you’re looking to meet new people - or get some advice or support for a friend or family member? Or you might be wanting to find out about exercise or social clubs, or how to get involved in a local charity?

Community services can provide specialist support too on a range of topics, such domestic abuse, helping to manage your finances, housing issues or bereavement support.

We can support older people and their carers to improve your health and wellbeing by:

  • Connecting you to local community services and activities, and
  • Helping you to reach your goals and ambitions.

This service is funded by Bromley Clinical Commissioning Group, and managed and delivered by Community Links Bromley, on behalf of Bromley Third Sector Enterprise.

It is part of a wider NHS programme of integrated care provision in the Bromley borough.

Would you like some 1-1 support to decide what services may be suitable for you?

The local Simply Connect Bromley team can help you to find out what issues you’re struggling with, and refer you into local support services that can help you.

This is known as a social prescribing service, and is something that NHS England have recently funded as an extension of the services provided by your local GP.

New Link Workers have been introduced to assist GPs in caring for a person’s health and wellbeing.

These workers (along with other health and social care professions) can help you access non-medical services that can really help you with whatever issue you are experiencing, that doesn’t necessarily need a medical intervention. This could be specialist support such as managing your health condition, bereavement support, keeping active or counselling services.

What is social prescribing?

This is when you are referred or signposted into non-clinical services by a professional as a way of supporting or improving your health and wellbeing.

Social Prescribing Link Workers are new posts, funded by the NHS, who will support you and make referrals on your behalf into community services.
