Care Navigators work with Clinical, Medical Therapy and Social Care professionals, to provide patients with a more integrated service. 

The aim of the integrated model is to ensure that services and professionals work together to provide an all-in-one service, which is more responsive to the needs of the people, is focused on preventing ill health and proactively managing and supporting people in a more co-ordinated way.

GP referrals via the three Integrated Care Network Hubs (Beckenham, Bromley and Orpington) in the Bromley Borough.

Frailty Pathway

Referrals from Multidisciplinary Team professionals at the Frailty Unit, Churchill Ward, Orpington Hospital

The Care Navigator is pivotal in capturing the patients non-medical support needs, navigating them through the system, identifying other support required and provides information and sign posting accordingly.
